HDD problems (possible OT)

I have been having intermittent problems with my computer. Every so often it just hangs mid process. I have tried all sorts of tweaks to no avail but i have just discovered the following in my dmesg

Is this an isolated hardware problem and should i just trash the disk and buy another or is it something i can sort out with some sort of setting? The problem is persistent across various kernel versions. I am running hdparm as a default service with the following settings in my /etc/conf.d/hdparm
