Would I be right in thinking that on-going maintenance of Gentoo involves a regular ... how often? ... emerge rsync and then emerge and updated packages? Being firmly rooted in the "if it ain't broke don't fix it" camp I've got a nicely running web/ftp/ssh server and don't feel the need to change but if and when the need arises it'd be nice to know what's needed.

There is no "right way" to do maintenance. As you said, if it isn't broken, then there's no need to fix it. Obviously, security patches might have some bearing on your upgrade frequency, but even then, the final decision is up to you.

Coming from a debian world, there are lots of people still using hamm and slink, which are ancient releases, but they do the job that they were built for.
