does anyone know what kernel config options are changed specifically when using the gentoo linux-headers patchset?

Likely none, because it is a patchset for kernel headers, not kernel sources (which include makefiles and Kconfig files), but if in doubt, download the patches archive, use tar Jxf <archive name> and find out...
