In the last 2 weeks, after a Plasma/KDE update, a stable Plasma-X11 on a Kaveri APU with two monitors started behaving in a rather broken way.:

1. Secondary monitor on HDMI (RH) started displaying the wrong resolution and remained stuck on Plasma splash screen.

The primary monitor on DVI (LH) loaded fine, which allowed me to go into systemsettings, disable the RH monitor and then re-enable it. This is something I had to do each time to be able to get the RH desktop to load. When I re-enable the RH monitor I get this in Xorg.0.log - no idea if it is relevant:

2. In the last two days, after a mesa and libdrm update, the LH monitor is now also getting stuck on the Plasma splash screen, albeit with the correct resolution. The only way I have found to get the desktop to load is by dropping into a VT and back again with Ctrl+Alt+F1/7. I still have to dis/enable the RH monitor to get it to load at the correct resolution.

I also noticed if I logout and restart the Display Manager sddm, then go through the above resetting gymnastics to get the desktop to load, the RH monitor's wallpaper is changed to the default 'Milky Way'.

Looking through dmesg and logs I can't see anything amiss. I've reseated the DVI and HDMI connectors, but it made no difference. I tried Plasma on Wayland and it loads correctly. I also tried Enlightenment-X11 and it also loads as it should. So this seems to be related to Plasma on X11 only. Have you come across anything related to this problem? Is there a fix?

I have no amdgpu experience, the only things that caught my attention in your xorg.0.log are quoted below. Which has me wondering if you require a /etc/xorg.conf file? Perhaps monitor sections for the outputs are required. The second quote shows a comparison between yours and my nouveau where mine shows a resize.

Actually there was something else, I was also getting freezes, applications wouldn't launch etc. Would reboot and sometimes worked for a minute or so.
I do regular backups with Image for UEFI, been using it since forever, came to find only recently every time I do a backup image of the Plasma partition, I need to boot into Gnome and do a filesystem repair from Disks utility. This gets rid of all the issues not related to firmware.
So....
It's odd, but I've been using KDE/Plasma since forever and this is no surprise that a filesystem touch up does the trick. No issues with it's Gnome twin, they use same kernel config, so it's not X11

The graphics firmware for my APU is loading correctly - dmesg shows no warnings or errors.

The difference you noticed in xorg.0.log is because I have two monitors connected to the card, with the secondary monitor on the right hand side of the primary monitor. Therefore the total width is double yours, 1016 x 285 Vs 508 x 285.

I've run btrfs scrub and btrfs check, both show no discrepancies with the filesystem. Plasma has been running on this hardware for more than 6 years and never has it experienced a problem like this before. Whatever is causing this problem seems to be related to a Plasma/KDE update and only manifests on Plasma-X11, not on Plasma-Wayland, or when launching other DEs on X11 like e.g. Enlightenment. Reverting the Plasma update to prove the point, if the older versions are still in the portage tree, is a pain and whatever the outcome it won't actually fix the problem - sooner or later I will need to update the system.

Yes I agree, it's Plasma

The only difference between my Gentoo Gnome and Gentoo Plasma is the DE, every thing else is identical and the file system is ext4.
I've been using KDE since Mandriva some 20 years now and every now and then it get's wonky after upgrades and most often in the past was to wait till future upgrades stabilize it or find a temporary workaround.

Eventually I found it easier to just copy a few files over to the NTFS Data drive, make a backup image of the partition and just format it and reinstall, since I don't keep data in the OS partition. For most pre-compiled Linux it's about 2 hours to build from scratch complete with personal touches using the same latest version packages that broke the old installation. Most often it runs perfectly taking this route, no putting up with the issues for a week or two before eventually just doing the reinstall. And sometimes, that's how I figure out what "I" did wrong with my previous installation.

But I did have KDE issues in the past using this same backup software where I couldn't boot the restored image and resorted to dd images for that OS, was like that for about a year before my backup software images would boot again. So, it only took a couple weeks of this recent wonky spell to figure out the current workaround on my end when I put the file system repair from Gnome and the backup images together.

Another issue with Plasma I've found over the years is the constant changes in the settings menu, and many times after a major version upgrade, some/all your personal settings have been neglected and it's set to it's current defaults. I went half a year with Debian KDE not able to switch desktops rolling the cube with the mouse scroll wheel because there was a default setting buried somewhere else other than the "Desktop Effects" section that needed to be flipped to the other setting before Desktop Effects worked properly. All the other bull I tried for a couple weeks including reinstall and everything....Yeah, a little choked after finding this new setting or a setting I didn't have to fiddle with in the past was the cause of a few weeks of trouble shooting.

EDIT: Although not related to your issues, another Plasma default setting I hate, on a new install, the Search feature does a file indexing, it will go through every drive mounted to put together an index of every file it finds so that when you do a search, the results pop up fast. This uses a ton of resources every time you boot the computer when you have lots of data and keeps Plasma kinda retarded for a while. One of the first things I do after a Plasma install is to disable Search file indexing.

OK, now Plasma Wayland is borked too. All I get is a black screen. Ctrl+Alt+F1 and back to F7 shows a mouse cursor which I can move around the black screen - but nothing else.

I've been having similar problems.

Loading plasma (X), loading screen appears, screen freezes. But if I let my computer sit, the lockscreen will come up and I can interact with it normally. Once it goes away, problem persists. Don't know of any workaround for me, I only have one video out port (hdmi). and switching to a TTY and back does nothing.

Using a similar apu, the gpu part is liverpool (ps4) and this is on a brand new install. This happened the first time I tried to install plasma. Wayland doesn't work for any DE I've tried so I can't speak for that. But, I have narrowed down the problem to Kwin. If you launch KDE with a different WM (for me, I am using i3) it seems to work fine. If you want a temporary fix to use a normal-ish plasma, I have heard openbox works with plasma fairly well. Kwin does handle a fair amount though.

exec env KDEWM=/usr/bin/i3 /usr/bin/startplasma-x11 #plasma with i3
in my .xinitrc and start with startx

After recent update I also had not so severe, but some issues with plasma. When I go out and login again, sometimes kwin crashes, the system loads, but with disabled opengl compositor. Usually second logout/login cures it. This happened both on intel laptop and nvidia proprietary codec desktop. Not sure how persistent it is, since I do not restart KDE session too often.

I have also seen (after hibernation) black screen with mouse coursor only, which ocassionally (but not always) was corrected by going to text console and back.
I'd say before this summer I never had any issues with plasma (well, at least in over 2 years)

If you've had similar problems with NVidia then this is not an AMD/Radeon APU bug as I thought it might be, but it seems related to kwin.

I have been using OpenGL-1.3 as the compositor, under SystemSettings > Display > Compositor. However, when I click Defaults it changes to OpenGL-2.0. I thought this may have been the cause of the problem, so I changed it to 2.0, applied and rebooted, but the same problem persists. Thankfully this is the only PC with this problem so far.

Yes, and visual delay on splash screen was because kwin was crashing in the background

Try alt+F2
it should open krunner normally. After fixing other issues on my system, this works and I can open programs with krunner. Desktop is still black, no desktop right click menus, no panels.
Also, if this works for you, try opening KDE's settings (systemsettings5) and tell me if the colors option persists. It doesn't for me, and it's probably not related but you never know.

Just going to drop some general experiences here in case they are useful.
  • KDE upgrades can be broken by existing configs.
    Create a new user and try logging in with it. If it works the process to fix your user becomes moving KDE related config files to a backup directory until the problem one is found, or replacing your's with the "default" files created in the new users ~/.config directory.
  • kscreen has been a shitshow for a long time, causing all manner of odd behaviours, and undesirable behaviours that can't be configured out.
    Personally, I keep it disabled. Anything it would have done at login I have covered by a working /etc/X11/xorg.conf.
    Even on the laptop, if I have to connect to an external screen/projector etc. it's easier to enable it as needed than deal with it's nonsense on a daily basis.
    The setting is in system-settings>startup and shutdown>background services>kscreen2
  • xrandr can obfuscate a broken xorg.conf
    Adding
    Code:
    Section        "Extensions"
           Option                  "RANDR" "Disable"
    EndSection
    forces X11 to use the config and not try any automagical resolution and screen set-up chicanery.
    Best to comment it out once the relevant config sections are working though; no one wants to reconfig and restart X just to add a temporary monitor.
