Sometime about 2003. Of course I also remember when we first got cable there were no commercials, back in the '70s.

Regarding cable and satellite pay TV in the US, the content creators pressure the carriers to carry certain channels in certain tiers. The carriers then squawk about how much and in what tier less popular / new / whatever channels will be offered. The more a content creator demands be in the lowest tier, the higher the cost of that tier. From a carrier's perspective, there is an advantage to having a visibly "low price" on the lowest tier.

The problem is mostly with content creators, not the carriers (satellite, cable).

Example: Viacom 2010s.

Go ahead, cut your cable. From an absolute cost perspective, that may make sense. But don't think that (paid) streaming means you're not subsidizing content you don't like or don't watch.

I have never had cable or satellite service. I bought my first house in 1997 and just didn't see the point in either.

As a matter of fact I installed two aerial antennas and hooked them up to my media server. I get 36 digital channels FTA. Plenty to watch, some motorsports I follow were not on cable anyway.
