I would like to create a custom US keyboard layout in order to type phonetic English
using orthography of Standard Simplified Sound Symbols (S4) http://034d0ec.netsolhost.com/S4wordpress/sample-page/
which adds glyphs for th/dh, sh/zh, and a couple new vowels.

I created a new keyboard layout:

I also created a new file /usr/share/X11/xkb/symbols/S4 based on that for us

But my new S4 keyboard is not showing up as an option in KDE->system-settings->input-devices->keyboard->layouts->add
