I've had a very bad fsck with kernel 4.0.4. It has raid and multipath enabled although no raid is set.
Am I the only one? I know my conf is weird but it's always better to ask.

There's a bug with linux >4.0.1 + EXT4 + software raid that includes filesystem corruption. Downgrade to 4.0.1 for the time being. Further details on phoronix.

Finally my issue was very likely to have a different (human) cause.
