To use QEMU with virt-manager I had to enable the IPv4 netfilter (IP_NF_IPTABLES, IP_NF_FILTER, IP_NF_TARGET_REJECT, IP_NF_NAT, IP_NF_TARGET_MASQUERADE and IP_NF_MANGLE specifically) in the kernel, otherwise it throws an error and I can't start a VM. However, I don't see any mention of this on the wiki. Did I do something wrong or is the wiki incomplete?

QEMU does NOT need NETFILTER in the kernel. Neither QMEU itself, not the Gentoo ebuild.

NETFILTER is required by the libvirt ebuild IF the USE flag virt-network is enabled.

If you want to use libvirt without NETFILTER activated in the kernel, disable USE flag virt-network for libvirt. Some features of libvirt won't work - but if you don't need those features, it's perfectly fine to emerge libvirt without the USE Flag virt-network.
