Pulseaudio: sounds not mixed but queued

I have a global pulseaudio USE flag set. In a GNOME session, I have mplayer playing a video and ktorrent downloading a file. During the playback of the movie, the download is completed and a notification pops up, but without a sound. After the video plackback finishes (i.e. the mplayer process is killed), I hear ktorrent's download completion sound.

What do I try to fix this?

Is pulseaudio set up and running in your gnome session?

Is gnome USING pulseaudio, or esd, or gst, or something else entirely?

Is your sound card supported by pulseaudio? I have a sound card that just does not work with pulseaudio, and is the reason that I asked. (though I haven't checked on it lately)
