It seems there is no Xvideo support for your video card available.
Run 'xvinfo' to verify its Xv support and read DOCS/HTML/en/video.html#xv!
See 'mplayer -vo help' for other (non-xv) video out drivers. Try -vo x11

no... flag "X" enable GUI for mplayer and not change anything for Xvideo support. Mplayer have a "xv" and "xvmc" flags for enable Xvideo support and this flags enabled for me.

If you are using the ati fglrx drivers make sure that you have set the video overlay to xv and that the permissions allow user access to the device. You may have to make sure that your user is a member of the group "video." You would use "aticonfig", included with the ati drivers, to modify xorg.conf appropriately, if you haven't already.

Also, I'm not sure how the ebuild works but when compiling mplayer manually, disabling X11 always disables xv and xvmc on my system. The gui is gtk2 so as long as you don't use that flag I don't think it will compile the gui. So it could be worth a try to enable the "X" flag.
