If you have a passive ISDN card you will have to use mm-sources or several patches from ftp://ftp.isdn4linux.de ...

I'm using linux-2.6.4-mm1 (patches already included) and it works quite smoothly.
I activated CONFIG_ISDN_I4L and compiled the whole thing into the kernel. devfs wasn't able to create the necessary devices under /dev/isdn so I created them myself:
# mknod c 45 128 /dev/isdn/ippp0
# mknod c 45 255 /dev/isdn/isdninfo
# mknod c 45 64 /dev/isdn/isdnctrl
# mknod ttyI6 c 43 5 <-
# mknod ttyI6 c 43 6 <- for vbox (virtual modem devices)
...
