Why are portage-utils and docbook-xml-dtd in /var/lib/db

I have just done a fresh install on an x86 system, I have made a few changes after which I have run
emerge -avef @system and @world

Looking in /var/lib/db I have portage-utils and docbook-xml-dtd which are reported as requiring downloads running emerge -pv for them. How can they be installed but not have the downloads completed as part of emerge -ef @system or @world?
