Hey, have you tried emerge without the N switch. All Rs are there because of the N switch.

You're right ... but emerge shouldn't do that, since there's nothing new about the newsflags (except that '%' symbol, I don't really get what that means..)

The same issue here. Seems to be portage-2.1.1_pre3-r5 bug. Just mask it and merge portage-2.1.1_pre3-r4

Its new feature since portage-2.1.1_pre3-r5, related especially with --newuse option :D ... if some USE flag is removed for package, portage will offer to reemerge that package. Same if some USE is added.

If some useflags are removed or added, but you never set these useflags active on your system. Why should portage offer to recompile these packages? It doesn't make sense ...

The percent sign following qt indicates that the qt option has been added to the package since it was last installed. 
But this (% sign) was introduced in portage some time ago (since version 2.1). But it was shown only when you was upgrading or manually reemerging packages. So this change you realized some time after this USE change was done. Now you know it "immediately" :( and portage offers now such packages to reemerge.

This feature is also present in the x86. The portage compares the IUSE os the ebuild to the IUSE of the installed package and if they differ then the portage tries to replace it but only when the N switch with the emerge is used.

But keep in mind, that before revison5, -avuDN never offers to reemerge packages with changed USE, which you dont have set anywhere (make.conf, package.use). I am syncing everyday and updating with command above, but never saw something like this - in terms of reemerging: (few lines from todays -avuDN)

The thing is, the IUSE has changed (it is a "newuse"), so when you want to update packages whose use variables have changed, Portage wants to update this.

I think you are right, at my system which runs a stable portage(2.1) tree the commands....

produce/show the same output(since its an update). And all the other packages which need replacement(due to the N switch) donot show the new use flags, so either they donot have the new use flags(and they are there becuase my /etc/portage/packages.use file is empty and I have USE="-*" in my /etc/make.conf) or you are right.
