Use package specified optimizations?

Should packages that specify their own optimizations use them, or should they only use Gentoo specified optimizations?

Isn't the whole point of Gentoo's flags in make.conf to override defaults? Don't we override defaults because the defaults usually assume that you have a 386 and want to use the following flags only:
"-O2 -pipe"
