I'm getting used to the larger cursor size in Xfree 4.3 however, rather annoyingly, when I go over the desktop icons in KDE, the cursor changes to the original Xfree circa 4.2 style hand pointer.

Is everyone else experiencing this same behaviour? Any one got any ideas on how to fix this?

Qt is not yet compatible with the new XCursors and uses on many occations the old ones. The only solution is to fix this in Qt.
There is already a patch from Stefano <stefanux@libero.it>: http://lists.kde.org/?l=kde-devel&m=104655590117552&w=2
(I haven't tested the patch and don't know if it actually works or if it introduces any new bugs)

AFAIK there exist no official solution from Trolltech yet.

Actually, the patch is unecessary - here is a good overview of the issue, and how to resolve it - it's simply a matter of making some sym-links in your cursors directory:

The cursors live under:

Code:
/usr/X11R6/lib/X11/icons


There are 4 directories within that contain the various cursor themes. That is where you would make the sym-links.

As for the cursor size, you can specify it in your .Xdefaults file.
IIRC, I've set it to something like :
Xcursor.size: 16
